Congratulations! You have worked hard to get here and your work has not gone unoticed. In order to get a NH Nursing Assistant License there are a few things you must do. All of the paperwork you will need was provided to you at orienation in your blue packet.
1. Submit a completed criminal record inquiry form and FBI Fingerprinting. The NH Board of Nursing (BON) must receive and review your criminal record report in order to complete the application process. The Board can only accept completed criminal record reports that are sent to them by the NH State Police.
To complete your criminal background check:
- Complete the top portion of the Criminal Record Release Authorization Form
- Have the form notarized (most local banks have notaries or a representative from LNA Health Careers can notarize it for you) The bottom section will need to be completed by a Notary.
- Call the State Police at 603-271-2538 to schedule an appointment at the State Police Headquarters in Concord to get your LiveScan Fingerprints taken. The fee is $54.25. Give the State Police your notarized criminal record release authorization form. They will send your results to the BON.
2. Once you have completed the course and received your certificate you will then be eligible to take the State Exam. Approximately 7-10 business days after you take the State Test, you will receive official results of your test either by mail or by email from the American Red Cross. Once you receive those you can then apply for your license.
3. Once you have received your test results, make a copy, make a copy of your certificate (which you received on your last day of class) and completely fill out the Board of Nursing application (follow along section 1). Be sure to fill in all required fields. You will also need to include a $35 check made payable to "Treausurer, State of NH." Mail all paperwork and the fee to the NH Board of Nursing at: 21 South Fruit Street, Suite #16, Concord, NH 03301.
4. Your license will be posted on the Board of Nursing website (http://www.nh.gov/nursing) within 2-3 weeks of submitting your paperwork. NO paper copies are sent out. Visit the BON website and click on the "online verification" section to view your posted license.
You are required to complete 12 contact hours every year to maintain your license and will need to reapply every two years (your license will expire on your birthday) for a renewal. You must also work a minimum of 200 hours every two years to stay active in practice.
You are required to notify the board if you move, or your name changes. Never give your license number to anyone you do not know over the phone. Numbers and licenses can be stolen and someone could practice under your number.
