From the North: Take 93 South to 293 South toward Manchester. Take Exit 5 (Granite Street Exit). At the end of the exit ramp take a left onto Granite St. Granite St turns into Lake Ave after you cross Elm St. Continue on Lake Ave (the Verizon Wireless Arena will be on your right). Take a left onto Pine St (Lake Ave turns into a one-way at this intersection and a left turn is your only option). Turn left onto Concord St. We are 1 1/2 blocks on your right. Parking is available on the street in 2 hour increments or in the Victory Parking Garage on the corner of Chestnut and Amherst. To get to the parking garage, turn left onto Chestnut Street from Concord Street. The parking garage will be on your right. *There is a 75 cent fee per hour to park in the parking garage.

From the South: Take 293 North to Exit 5 (Granite Street Exit). Wellington Rd turns into Bridge St at Mammoth Rd. Continue on Bridge St until you reach Chestnut St. Take a left onto Chestnut St and then a right onto Concord St. Parking is available on the street in 2 hour increments or in the Victory Parking Garage on the corner of Chestnut and Amherst. From Manchester/Concord: From the Amoskeag rotary in Manchester (off exit 6), take the exit towards Goffstown onto Goffstown Road. Follow Goffstown Road for several miles, staying to the left at the split. Goffstown Road becomes Goffstown Back Road. You will come to a small rotary. Go around the rotary as though you were going straight through it. Continue on for less than a mile and the facility will be on your right.

From Manchester: Take 293 to the Kilton Rd exit. After you have passed the shopping center on your left take a left at the Intersection. Follow this road through 1 set of lights. At the second set of lights take a left. Go up the hill, past the shopping center.

[+] Aurora Senior Living of Derry (formerly Derry Healthcare & Rehab Center): 20 Chester Rd, Derry, NH 03038

From Manchester: Take the Exit 4 (Londonderry exit) from I-93. Turn left off of the exit ramp. Follow this road through town. At the traffic circle, go half way around (ending as if you cut straight across), to the opposite exit. This is Chester road. Less than a half mile on the left hand side of the road on the hill is Haven Health Center.

From Nashua/Hudson: Take the 102 all the way through town into Derry. When you come to the traffic circle, go around and take the exit that would be as if you were to go straight through the circle. This is Chester road. Less than a half mile, on the left hand side of the road, is Haven Health Center on a hill.

From South: Take Route 28 North, the facility will be located on the left, just before the traffic circle.

From North: Take Route 28 South to the traffic circle, take the 2nd right on Route 28 South. The facility will be located on the right.

From West: Take Route 4 to the traffic circle. Take the 1st right onto Route 28 South. The facility will be located on the right.

From East: Take Route 4W to the traffic circle. Take the 3rd right onto Route 28 South. The facility will be located on the right.

From Keene: Take NH-101 East for approx. 4 miles. Turn right onto Jaffrey Rd (NH-124). Follow this for approx. 13 miles. Turn right onto Fitzgerald Drive. Plantation Drive will be on the left after a 1/2 mile.

From Peterborough: Take 101 to 202. Follow 202 for 5 miles and turn left onto Hillcrest Rd. Turn left onto NH-124 and right onto Fitzgerald Drive. Plantation Dr will be on the left.

From Manchester/Merrimack (North): Take the Everett Turnpike to Route 3 South. Take exit #4. At the end of the exit ramp, turn right onto East Dunstable Road. At the lights, take an immediate right onto Harris Road. The facility will be on your right.

From Mass. (South): Take Route 3 North to exit #4. At the end of the ramp, turn left onto East Dunstable Road. At the lights, take a right onto Harris Road. The facility will be on your right.

[+] Hillcrest Terrace: 100 Alliance Way, Manchester, NH

From South: Take Route 293 to Exit 7 (Rt. 3A North). Go approximately 1.4 miles to Hacket Hill Road. Proceed approx. 1/4 mile to Countryside Blvd. Turn left and go 1 mile. Make a right onto Alliance Way. First entrance is for Pearl Manor and the third entrance is for Hillcrest Terrace.

From North: Take Route 93 South to Exit 10 (do not take Hacket Hill Road. Exit at Hooksett tolls. Go through toll booth). Go left onto Rt. 3A South. Go .7 mile and make right turn at traffic light onto Hacket Hill Road. Proceed approx. 1/4 mile to Countryside Blvd. Turn left and go 1 mile. Make right onto Alliance Way. First entrance is for Pearl Manor and the third entrance is for Hillcrest Terrace.

Take Route 3 South: Exit #1. At the end of the exit ramp, at the lights, take a right. The facility will be on the right hand side, less than 1/2 a mile down the road. There is a large iron gate at the entrance. The driveway is long and you can not see the facility from the road, but there is a sign at the entrance.

From Somersworth: Take NH-9 for approx. 3 miles. Turn left onto NH-108 (Central Ave). Follow Central for approx. 2 1/2 miles. Turn left onto Court Street. Court Street becomes Middle road after about .5 miles down.

From East: Take Route 101W. At intersection for Routes 9, 10 and 12, go straight. Continue through a flashing yellow light and take the next right onto Whitcomb Mills Road. This will take you to the rear entrance of the facility.

From West (Brattleboro area): Take Route 9 (Franklin Pierce Hwy). Turn left onto Whitcom Mills Rd. Take first right onto Arch St. Facility will be on your right.

From Concord Area: Take 93N to 89N. Take exit 5 to Route 9 West. Continue on 9W into Keene. You will come upon a set of lights, turn right to continue on Route 9W. Go through a flashing yellow light and take the next right onto Whitcomb Mills Rd. This will take you to the rear entrance of the facility.

[+] Langdon Place (Nashua): 319 E. Dunstable Rd, Nashua, NH

From South: Take Route 3 North to Exit 1. Take a left off the exit ramp onto East Dunstable Road. Facility will be a 1/4 mile up on the left hand side.

From North: Take Route 3 South (Everett Turnpike) to Exit 1. Take a right off the exit ramp onto East Dunstable Road. Facility will be 1/4 mile up on the left hand side.

From Merrimack/Bedford/Nashua: 293 North, Exit 6 (Amoskeag Bridge Exit) go across the bridge and turn right at Elm St. continue South until you come to Bridge St. and turn Left onto Bridge St. Follow Bridge St. until you come to a 7-11 store with a light. At that light turn left onto Maple Street. Once on Maple St. turn left onto Pearl Street and go about 2 blocks. The facility will be on the right.

From Manchester/Concord/Hooksett: Take 93 South to Exit 8 (Bridge St. exit); turn right onto Bridge St. off the exit ramp. Follow Bridge St. down the hill. On the right hand side is a 7-11 store. At the light turn right onto Maple Street. Once on Maple St. turn left onto Pearl Street and follow it for about 2 blocks. The facility will be on the right hand side.

From North (93): Take 93 South to exit 14. At the end of the exit ramp, turn right onto Loudon Road. Turn left onto North Main Street. Turn right onto Pleasant Street. Facility will be on the left. Once you have turned into the facility entrance, PV Genesis is the building on the right.

From North (89): Take 89 South to exit 5, for Route 9. Turn right onto 9E. Follow for about 7 miles then turn right onto Pleasant St. Facility will be on the left. Once you have turned into the facility entrance, PV Genesis is the building on the right.

From South: Take 93 North to exit 14. At the end of the exit ramp, turn left onto Loudon Road. Turn left onto North Main Street. Turn right onto Pleasant Street. Facility will be on the left. - OR - Take 93 North to 89 North. Take exit 2, turn right onto Clinton St. Take the first left onto Langley Pkwy and the first right onto Pleasant St. Stay to the right onto Pleasant St. and facility will be on your left. Once you have turned into the facility entrance, PV Genesis is the building on the right.

From North (93): Take 93 South to exit 14. At the end of the exit ramp, turn right onto Loudon Road. Turn left onto North Main Street. Turn right onto Pleasant Street. Facility will be on the left. Once you have turned into the facility entrance, PV Retirement Center is the building on the left, facing the street.

From North (89): Take 89 South to exit 5, for Route 9. Turn right onto 9E. Follow for about 7 miles then turn right onto Pleasant St. Facility will be on the left. Once you have turned into the facility entrance, PV Retirement Center is the building on the left, facing the street.

From South: Take 93 North to exit 14. At the end of the exit ramp, turn left onto Loudon Road. Turn left onto North Main Street. Turn right onto Pleasant Street. Facility will be on the left. - OR - Take 93 North to 89 North. Take exit 2, turn right onto Clinton St. Take the first left onto Langley Pkwy and the first right onto Pleasant St. Stay to the right onto Pleasant St. and facility will be on your left. Once you have turned into the facility entrance, PV Retirement Center is the building on the left, facing the street.

From North: Take 93 South to exit 14. At the end of the exit ramp, turn right onto Loudon Road. Turn left onto North Main Street. Turn right onto Pleasant Street. Facility will be about a mile down the road on the right.

From South: Take 93 North to exit 14. At the end of the exit ramp, turn left onto Loudon Road. Turn left onto North Main Street. Turn right onto Pleasant Street. Facility will be about a mile down the road on the right.

Take 93 North to exit 14. At the end of the exit ramp, turn left onto Loudon Road. Turn left onto North Main Street. Turn right onto Pleasant Street. Facility will be on the left.

From Manchester: Take 93N to exit 20. Turn left onto RTE 3 (Daniel Webster Hwy). Follow this for about 6 miles and the facility will be on your right.

[+] St. Teresa's Manor: 519 Bridge St, Manchester, NH 03104

From Any Direction: Take I-93 North/South to Exit 8 (Bridge St. exit), and turn right onto Bridge street. Follow this street and soon you will cross over Mammoth road. On the left hand side you will see Trinity High School. Right after the school, there will be signs for St. Teresa's Manor on the left.

From North: Take 89 South toward Concord. Take the RT-13 N exit, Exit 2 toward Concord. Take a slight right onto RT-13/Clinton St. Go 0.9 miles, turn left onto Langley Pkwy. Take the 1st left onto Pleasant St and go 0.3 miles. Facility is on the right.

From South: Take the Everett Turnpike North toward Manchester/Concord. Everett Turnpike becomes I-93 N (portions toll). Go 2.3 miles and then merge onto I-89 N. Take the RT-13/Clinton St. exit, Exit 2, toward Concord and go 0.3 miles. Turn right onto RT-13/Clinton St. and go 0.7 miles. Turn left onto Langley Pkwy and go 0.8 miles. Take the 1st left onto Pleasant St. Go 0.3 miles before reaching facility on the right.

[+] Villa Crest: 1276 Hanover St, Manchester, NH 03104

From Manchester/Nashua: Take 93 to exit 6 (Hanover St exit). Go straight through the first light and turn right at the second light. About a fourth of a mile down the road you will see the facility sign on the left.
